We're seeking a Registered Nurse (RN) to join our team in a dynamic hybrid, case management role supporting individuals in both Residential and Day Programs. In this position, you'll coordinate and oversee medical services, ensuring each individual receives high-quality, person-centered care. The RN will play a key role in assessing health needs, managing care plans, and collaborating with interdisciplinary teams, while also providing training and guidance on health-related topics to both individuals and staff. Reports To: Supervising R.N.
Physical Requirements: Walking, bending, pushing, pulling, climbing stairs, lifting, etc.
Hours of Work: Full time, 37.5 hours per week Monday thru Friday. Rotating on-call, evenings, weekends and holidays, as needed. Shifts and hours may vary depending on agency need. Hybrid Remote Schedule working 3 days from home.

The overall responsibilities of the RN position focus on providing medical services to individuals in Residential and Day Programs. The RN is also responsible for providing training and instruction in health areas as requested to individuals and staff.
